Hospice / Pallative Care Palliative Care is "the active total care of patients whose disease is not responsive to curative treatment."* The goal of palliative care is the achievement of the best possible quality of life for patients and their families during this stressful time of life. Cornell Pharmacy has dedicated itself to providing the best palliative care services for hospice agencies, focusing on meeting the needs of patients at the end of life. Our pharmacist attends interdisciplinary team meetings once a month to foster better relationships between the nurses and pharmacist as well as the medical director. In addition, our pharmacist is available 24/7 to help answer tough medication questions when admitting new patients and when patients start to decline near the end of life. Cornell Pharmacy works with the patient and practitioner to solve problems by customizing medications that meet the specific needs of each individual. We guarantee 2-hour turnaround time for stat or emergency medications and provide personalized delivery service by employing our own drivers. Our drivers use cars that are clearly marked with the Cornell Pharmacy logo to insure that patients and their families feel safer when accepting evening and late night deliveries. Our staff knows how difficult it can be for the families of end of life patients. Our goal is to provide the highest level of support possible to hospice agencies, their patients and families. *World Health Organization, 1990 |
